The rules, window by window

Windshield

Windshield tinting is prohibited unless it was incorporated by the manufacturer and complies with the Canada Motor Vehicle Safety Act, or it is applied to no more than the top 127 mm of the windshield and does not reduce light transmission below 25% (uniform shade band) or below 5% in the top 64 mm and 25% in the remainder (graduated shade band).

Reflectivity

A window to the immediate left or right of the driver's seat must not have tinting that increases light reflection above 35% (s.3.22(2)(b)).

Medical exemption

No medical exemption provision identified in s.3.22 of the Vehicle Equipment, Safety and Inspection Regulation. Not confirmed either way from a primary source in this pass.

Enforcement

An inspection station or authorized person may affix a mark verifying that a window's light transmission/reflection complies with the regulation (s.3.22(4)); compliance is measured with a test meter operated per manufacturer instructions (s.3.22(5)).

Confirmed directly from the regulation text (Man Reg 31/2019, s.3.22, 'Tinted windows') via CanLII. Manitoba's rear-window rule is unusual among the provinces reviewed: for a passenger car or multipurpose passenger vehicle, tinting on ANY window (which in context includes the rear window) must not reduce light transmission below 50% UNLESS the vehicle has outside rear-view mirrors on both driver and passenger sides (s.3.22(3)), in which case the 50% floor does not apply to that window. No explicit numeric limit was found for rear side windows (behind the driver) specifically, distinct from 'the rear window' language in s.3.22(3); this entry treats rear side windows as unrestricted based on the absence of a specific provision, but this should be flagged for confirmation given the overlapping wording between 'window' generally and 'rear window' in s.3.22(3).
